Problems of achieving a satisfactory erection are fairly common. If the problem does not originate from lack of lust and is great enough to make normal intercourse impossible, the condition is called erectile dysfunction (impotence). Erection problems are caused by combined physical and psychological factors.

THE ERECTION MECHANISM

The penis has three bodies containing a dense network of blood vessels along all its length. During erection blood is filled into the vessels of these bodies. The bodies then engorge, make the penis hard and rise it up. The filling of these bodies occur when the vessels leading blood to the penis relax and vessels draining the penis constrict their volume.

Before and during erection nerves lead impulses to the genitals. The nerve ends in this area then releases the substance nitrogen oxide. Nitrogen oxide will diffuse through the genital area and the penis and stimulate the reaction of the blood vessels in the penis.

PHYSICAL CAUSES OF ERECTION PROBLEMS

Erection problems can occur because of disorders in the nervous system sending impulses to the genital area, problems with the blood supply to the penis and anatomical problems in the penis or genital area. Specific causes can be:

- Accidents, stroke, surgery or tumours hurting brain areas or areas in the spinal cord responsible for erection impulses.
- Multiple sclerosis, a disease hurting the isolating sheets around the neural fibres in the brain and spinal cord, can give erection problems.
- Accidents or diseases hurting nerves from the spinal cord to the genital region.
- Atherosclerosis caused by age or an unhealthy lifestyle, giving narrowing and hardening of blood vessels to the genital region.
- Injury to the erectile bodies caused by inflammation, accidents or diseases.
- Congenital malformations in the penis or genital region, for example hypospadias and epispadias.
- Peyronie's disease, a common inflammatory disease causing abnormal bending or twisting of the penis, and sometimes also hinder the filling of blood into the erectile bodies, sometimes gives problems for the erections.
- Circumcision causing the penile skin to be too tight, or causing extensive inelastic scars.
- Side effects of medicines, such as medications taken for high blood pressure or depression.
- Zinc deficiency.
- Heart disease.
- Diabetes causing injury to the nerves and blood vessels to the penis.

. Liver or kidney disease.
- Alcohol or drug abuse impairing psychological and neural functions.

PSYCHOLOGICAL CAUSES OF ERECTION PROBLEMS

Psychological causes can interfere with the erection process by distracting a man from stimuli that normally would give him sexual arousal. Psychological issues account for about 40% of erection problems. Erection problems in men under age 50 are most likely to be caused by psychological factors. Psychological causes of erection problems include:

- Anxiety for not being able to perform sexually as well as the partner demands.
- Prolonged emotional upset, such as worrying, anxiety or anger due to a man's economical, professional or social situation.
- Relationship problems, for example when the woman has different preferences of sexual practice than the man.
- A man who looses sexual desire for his partner may develop erection problems.
- Recently widowed men can get erection problems.
- Some men have difficulty having sexual intercourse with their partner after she has given birth because he does not like the changes that the birth process has caused to the woman's body.

CAUSATIVE TREATMENT

When a specific organic and psychological condition causes problems for the erections, this problem should be treated with appropriate methods that will vary according to the actual disease.

If the cause is heart disease or atherosclerosis, lifestyle changes and training adapted to the medical situation can often improve the general health situation and improve the erectile abilities.

If the cause of the erection problems is a problematic penile shape, for example curved or twisted penis by Peyronie's disease, using a mechanical penis reaction device for some time may help.

    Hi There

    Please answer my question only if you understand well what the problem is and you know the solution that will suit the best. I live in UK and would have to go via my GP to some GUM hospital under NHS.

    I am 29 years virgin male i.e. no sexual intercourse experience with girl and no anal sex either. For reasons beyond my control, I did not come to know that the penis foreskin should retract back normally till I was 28 years of age. I had mild phimosis for which I used tropical steroids and manual stretching after which skin started retracting in the penis while not erect. But when my penis came in erect state only after hard pulling (quite uncomfortable) it was possible to get skin retract. But then I discovered something very strange, my foreskin I guess has not separated from my glans penis in the right age. I have no clue if it was natural or not. I suspect someone stretched it forcibly when I was child (up to age 9 years) or I did it while masturbation. I started masturbation at 9 years and doing till date on an average 4-6 times a day. Direction was always upwards and now doing it downwards as in normal men, I get uncomfortable. Please note I never experienced any pain while masturbating with foreskin above glans penis in erect state or when both got separated forcibly.

    Complication is that penis errection dysfunction which is caused by an inner lump of skin inside (2 cm in diameter) of foreskin. Glans penis on left side has got a same sized pit/scar, which did not heal as I can’t quit masturbation due to high sex drive . If I force foreskin to come back and keep penis erect, some blood starts to seep from pit and the lump sometimes. Then, penis becomes soft.

    I know circumcision can remove foreskin with lump problem removing any errection issue, but what will happen to pit on glans penis? Moreover I am scared of operation. Should I use condom for sex and forget operation or costly / complex treatments?

    Can someone experienced or having any idea of this problem's treatment advice me what to do? I do not want to get circumcised but an effective plastic surgery without pain or repairing my foreskin will be acceptable.

    Please can someone provide any help that's useful? I done enough research, but could not find answers.

    Please note there is no tearing of skin or frenulum anywhere. A drop of blood seeps from scar-pit on glans penis or inner skin lump of sliding foreskin. This lump is creating my penis foreskin not to retract fully and if done forcibly it gets uncomgortable or painful. Can you suggest if a plastic surgery operation can fix or make lump of inner foreskin thin. It will remove phimosis altogether. But, I do not want circumcision.

    if there's phimosis going on, there's an operation to loosen the phimotic band. Not an elegant solution to the problem, but it works.

    Often, the frenulum is another source of problems. You could have a frenulum breve. Again, a simple snip, and the problem is solved. From what you're describing, this is more than likely your problem. When you're seeing a bit of blood, it's the skin of the frenulum tearing. You may or may not feel any pain. If you keep tearing the frenulum, it's gonna get all scarred up, and make the problem worse.

    Circumcision would be the last option. Pretty radical. And it sounds like you really don't want this option.

    You could try viagra but I will tell ya what, that stuff sucks. The side effects are terrible. Have you ever tried a sexual enhancer?     From your description I would guess that you have a skin bridge (not the frenulum) as a result of the phimosis and the forcible reduction of the foreskin. Foreskin adhesions (the foreskin becoming stuck to the glans penis) can occur and be manually reduced, which it sounds like you did. Now, a pocket has formed due to this and when traumatized during reduction in some manner it will bleed out of that "pocket". Obviously you were never circumcised (or if you were the circumcision was incomplete - leaving too much foreskin) I would continue trying to pull the foreskin back and apply 1% hydrocortisone cream once per day to the "pocket" area for 7 to 10 days to try to break that portion of it back. The the area clean and dry. If that does not work, you may need surgical release of the penile adhesions, a dorsal slit (a separation of the foreskin to allow it to reduce), or a formal circumcision. This is the advice given by the urologists I have worked with for 13 years.

    It really does sound like something you should see your GP about as you are obviously in discomfort. It may not be necessary at all but personally I would not rule out circumcision as it is a minor half hour procedure - but do let your doctor take a look.

    You should simply ask your doctor for a referral to your hospital. There, you will have explained to you what procedures can be recommended.

    Do not be embarrassed about the nature of your enquiry. You have written an exemplary description of your condition, but remember these words: "For relief of pain." Include them into your request and you should speed things along. You certainly ae in need of relief of pain, anyway.
